Cascade Occupational Health is here to serve your workplace health needs. From pre-employment screening to physical therapy after a workplace injury, we are dedicated to helping you stay safe, healthy and able to work.

Below, you’ll find information and resources to help make your visit as efficient and pleasant as possible. If you have any questions or are not able to find the information you need, please call our offices before coming to the clinic. Some appointments require specific preparations or paperwork and if you have not followed required guidelines, we will have to reschedule your appointment. Thank you!

If you have an appointment, please arrive 15 minutes prior to your scheduled time to check in and complete any necessary paperwork.

If you cannot make your appointment, please cancel 48 hours in advance by calling (541) 228-3100 and choosing option 1.

You must bring:

A valid form of identification

Authorization for Services form (“Passport”) from your employer, if required (please check with your employer)

Any visit-specific paperwork (please see Preparing for Your Appointment, below)

Your health insurance card. This is required even if you expect workers’ compensation insurance to cover your visit.

When you arrive, you must check in. Our clinic is very busy, and we cannot monitor our lobby. If you do not check in, we will not know you are waiting, and you may miss your appointment. Please stand at the sign and wait to be called to one of the registration desks. We will check you in, review your paperwork and ask you to wait in our lobby until you are called back to an exam room.

Weapons-Free Zone

Weapons are prohibited on the premises of all Cascade Health facilities. No guns are allowed, regardless of whether an individual has a license to concealed carry. The only exception applies to uniformed police officers.

Mask Policy

At Cascade Health, the safety of our clients, patients, and employees is our top priority. Masks are no longer required in health care settings, but are encouraged in our clinics. If you are symptomatic, we may request that you wear a mask.

Proper preparation for your appointment will ensure that you are see as quickly as possible. In some cases, if you do not bring specific paperwork or materials with you, or do not come at a certain time, we will not be able to complete your exam and you will need to reschedule.
