Pay Range: $41.19 - 63.50

Job Summary:

The Hospice Admissions RN serves in a consultant role, acting as an advocate for hospice care and providing education for healthcare professionals.  The Admissions RN assists the referrals department in processing new referrals, meets with patients and families to explain the hospice benefit and facilitate their transfer onto hospice services.  This position covers Lane County, and requires travel throughout the region to perform admissions, process documentation, and contribute to the initial plan of care.  Maintains a high level of professional expertise and assumes responsibility for professional growth and development. 

Education & License Requirements:

Current licensure in the State of Oregon as a Registered Nurse and current CPR card.

Minimum Qualifications & Experience:

Hospice experience required.  Must have demonstrated clinical competency, ability to collaborate with others, ability to initiate and plan own work activities, effective patient/staff teaching skills, flexibility, and a high degree of written and verbal communication capability.  Must have a dependable, insured vehicle, a valid driver's license, and a means of communication.  Need to be comfortable with completing clinical documentation on a laptop computer.  Must have the ability to share on-call responsibilities for sick call and holiday coverage.  Will function as an integral member of a multi-disciplinary team. 

Essential Job Functions:

  • Inform eligible patients and their families about the hospice philosophy, goals, services, and explain Medicare/Medicaid and private insurance reimbursement.

  • Assess patients to determine their eligibility to elect the hospice benefit, based on diagnosis-specific hospice criteria.

  • Complete a comprehensive physical, cognitive, and functional assessment.

  • Develop the initial hospice plan of care, effectively addressing physical needs and pain/symptom management concerns while providing instruction to patients, families and caregivers.

  • Participate in new staff orientation.

  • Serve as a consultant to referral sources and the community regarding end-of-life care and hospice care reimbursement.  Act as a liaison and advocate for hospice patients.

  • Collaborate with physicians, nurses, social workers, and other medical professionals to ensure physical, psychosocial, and spiritual needs are met.

  • Facilitate the decision-making process, reinforcing collaborative relationships. 

  • Assist with research, CQI, QAPI needs.  Will complete electronic-based clinical documentation.

  • Support physicians and other community-based referral sources in early identification of patients that may qualify for hospice services.

  • Work independently utilizing good time management, organizing and prioritizing tasks.

  • Participate in conferences, staff meetings, and in-services. 

  • Use healthcare related equipment, perform CPR, and respond to emergencies. 

  • Negotiate terrain and physical obstacles to reach patients, drive vehicle safely.

  • Perform other duties as assigned.

Marginal Job Functions:

Answer phones.  Clerical duties.  Move furniture, equipment, and supplies.  Post and access schedule.
